  Because you are US citizen coming with US passport, you have to send your US CBC. However, if you have stayed in UK for more than 2 years, you will be requested to send UK CBC as well.

  You will have to notarized and apostille your PGCE since it will be same as your BA Diploma and instead of TEFL certi, you will have to send photocopy of your teaching certification that is not apostilled.
